Clinical Characteristics of Acute Bacterial Prostatitis

Abstract
For the characterization of clinical course in acute bacterial prostatitis, we analyzed symptoms, etiology, associated diseases, treatment, and subsequent course of patients with acute bacterial prostatitis. We reviewed clinical records of patients with a minimal follow-up of 6 months(mean 13 months) admitted to our department with the diagnosis of acute bacterial prostatitis. Mean age of enrolled patients was 58 years. Associated diseases were accompanied by 15 patients(9 benign prostatic hyperplasia, 4 diabetes mellitus, 1 benign prostatic hyperplasia with diabetes mellitus, and 1 neurogenic bladder) in total 29 patients. The clinical findings were dysuria(90%), fever(79%), perineal ache(62%), urinary retention(31%), and gross hematuria(24%). A tender(93%) and swollen(86%) prostate was the characteristic features by rectal examination. The most common pathogen identified in urine culture was E. coli. The antibiotic profile showed a sensitivity nearly all to cephalosporin and aminoglycosides. Urinary retention showed significantly long hospital days and administration duration of oral antibiotics. Also, presence of associated diseases showed significantly long hospital days. Conversion to chronic prostatitis was identified in 7 patients and prostatic abscess in 1 patient within 3 months of follow-up. Early diagnosis, prompt therapy, and subsequent follow-up are important in the management of acute bacterial prostatitis. Our results suggest that urinary retention and presence of associated diseases may require more careful treatment.
